Roger L. Kozak, 66, Vermillion, formerly of Lake Andes, died Wednesday, Aug. 1, 2012, at Sanford Vermillion Hospital.
He was born on Oct. 15, 1945.
Funeral services will be at 10:30 a.m. Monday at St. Agnes Catholic Church, Vermillion, with burial in Calvary Cemetery, Vermillion.
Visitation will start at 5 p.m. Sunday, with a prayer service at 7 p.m., at the church.
