Horizon receives recognition
HURON — The National Committee for Quality Assurance announced recently that Horizon Health Care Inc.’s James Valley Community Health Center, of Huron, has received special recognition.
The organization has been awarded Level 3 Recognition from the NCQA Patient-Centered Medical Home 2011 program for using evidence-based, patient-centered processes that focus on highly coordinated care and long-term relationships.
“Our providers and staff work hard to improve our patients’ understanding of their health and related conditions so they are able to take a more active role in their health care,” said Jeff Mengenhausen, COO of Horizon. “As a result, our patients may experience better health, fewer hospitalizations, improvements in transitions of care and greater engagement. The PCMH program is one more way we are moving from a system that rewards the quantity of procedures to a system that rewards quality outcomes.”
