The Mitchell facility received an overall five-star rating, a distinction that about 18 percent of nursing homes in the state received. It was also one of only two nursing homes in the state to receive five stars in all three categories.

The three rating categories were health inspections, nursing staff and quality measures for residents. Fire safety and any fines the nursing home may have faced in the past year were also taken into account for the rating.

"I was very excited and proud of the team I work with here," Avera Brady Health and Rehab Administrator Veronnica Smith said when she first heard about the rating.

Smith believes the facility scored so well because of the employees.

"It's because of the people who work here," she said. "They are invested in the mission of helping the residents and making their lives better."

Mitchell's other nursing home, Firesteel Healthcare Center, received an overall rating of two stars, with one star in health inspections and four stars in both nurse staffing and quality measures.

Four other nursing homes in the area received a five-star overall rating. They were Diamond Care Center in Bridgewater, Good Samaritan Society in Scotland, Lake Andes Health Care Center and Platte Care Center.

Nursing homes in the area that received four overall stars were the Golden Living Centers in Armour and Salem, Weskota Manor in Wessington Springs, Oakview Terrace in Freeman, Menno-Olivet Care Center in Menno, the Good Samaritan Society of Tyndall and Tripp, and the Avera Rosebud County Care Center in Gregory.

Three-star nursing homes in the area were Avera Bormann Manor in Parkston, Prairie View Care Center in Woonsocket, the Good Samaritan Society in Howard and Canistota, and Sanford Chamberlain Care Center.

Two star facilities were Aurora-Brule Nursing Home in White Lake, the Good Samaritan Society in Wagner and Corsica, and Winner Regional Healthcare Center in Winner.
